Boarding Admission Forms. All pets boarding with us must have a brief questionnaire filled out by their owners each and every time they board. This will allow you to tell us exactly how you would like you pets taken care of during their stay. One form must be filled out per pet. You may download the form here, print it and fill it out, and bring it with you at the start of your boarding stay. If this is your first visit to Hanover Veterinary Hospital, you will also need to fill out a new client form, available above.

Low-Cost Spay/Neuter Clinic for Cats. Hanover Veterinary Hospital will be hosting a low-cost spay/neuter clinic for cats to help area residents control the outdoor cat population in their area. Please download this registration form here, print it and fill it out, and return it to us at least one day before the clinic to reserve your spot (you do not need to fill out a new client form).
